Will my visit be private? How is my information used?

Your phone or video visits will not be recorded or saved

While the virtual medical office is private, the provider will need personal information so please find a private space in which you feel comfortable sharing that information; additionally, please ensure you use a secure and private internet connection

Your provider will tell you if someone else from their office can hear or see you

Your information will not be shared with anyone without your consent, with the exception of when
necessary for treatment (e.g. health information relayed to pharmacists or other physicians for the
purposes of coordinating care) or for billing (will not include personal health information)

The communication and electronic records system are HIPAA compliant and designed to protect your information

You have the right to copies of your medical record

You have the right to inspect and request changes to your information

These rights are protected by Federal and State law
